    Associate Director of Network Development and Engagement, Modernizing Senior Centers Resource Center and NISC

    Dianne Stone has more than 20 years of experience with senior centers and aging issues, primarily as the Director of the Newington Senior and Disabled Center in Newington, Connecticut. Stone has also worked, volunteered, and held leadership positions with a variety of organizations in Connecticut, including Connecticut AgeWell Collaborative, the ADA Coalition of Connecticut, Connecticut Medicaid Oversight Council, and the Commission on Women, Children, Seniors, Equity and Opportunity. She joined NCOA in January 2022, as a member of the Center for Healthy Aging, where she is primarily focused on supporting senior centers throughout the country through the ACL-funded Modernizing Senior Centers Resource Center and NISC.

    Join the webinar for a discussion of how the final rule may impact CBOs implementing, expanding, and sustaining evidence-based health and wellness programs through individual organizations or community care hubs (CCH).

    Every year, the Department of Health and Human Services’ Centers for Medicare & Medicaid Services (CMS) issues a Notice of Proposed Rule Making for the Physician Fee Schedule (PFS) that define services and reimbursement rates for physicians and other providers providing care to people with Medicare. As of Jan. 1, 2024, Medicare will reimburse for various services including Community Health Integration services, Social Determinants of Health risk assessments, and Principal Illness Navigation services. Furthermore, the final rule provided supportive language for clinical providers to contract with community-based organizations (CBOs) to meet the needs of beneficiaries.     Timothy McNeill, RN, MPH

    Freedmen’s Health

    Timothy P. McNeill is the founder of Freedmen’s Health, a Washington, DC healthcare consulting firm specializing in implementation of innovative models of care. Mr. McNeill has led the firm’s consulting efforts serving customers including the U.S. Department of Health and Human Services (HHS), U.S. Administration on Aging (AoA)/Administration for Community Living (ACL) and various Foundations and National Non-profit organizations. Under an HHS/ACL contract, Mr. McNeill has been the lead technical assistance provider to establish and support integrated networks to deliver new models of care that address medical risks and social determinants of health supporting value-based contracting in 26 States.

    Mr. McNeill has started or expanded multiple sustainable health programs including two Medicare Shared Savings Program (MSSP) ACOs, an IPA made up of FQHCs and independent physicians, a network of community-based free clinics, managed the operations of a network of Federally Qualified Health Centers, and established multiple regional networks to deliver Long-Term Services and Supports, contracting with MCOs, in support of State Medicaid Waiver implementation. Mr. McNeill is a Registered Nurse with a bachelor’s degree from Howard University and a Master of Public Health from Eastern Virginia Medical School. Mr. McNeill is also a retired U.S. Navy Nurse Corps Officer.

    Sara Clary, J.D.

    Consultant

    National Council on Aging

    Sara is a consultant with the National Council on Aging. Over the last 15 years, she has worked on improvements to the Medicare Part D Extra Help and other programs for vulnerable people at the federal level.

    Prior to joining NCOA, Sara was an Attorney with Legal Services of Southern Piedmont in Charlotte, North Carolina, where she helped people access available public benefits programs. 

    Sara received her Bachelor’s degree in Politics from Salve Regina University in Newport, Rhode Island and her J.D. from the University of Miami (Go ‘Canes!). She resides near Chicago with her husband 
    and four kids.

    Sophie Morgado

    Legislative & Policy Associate

    National Council on Aging

    Sophie Morgado serves as the Legislative & Policy Associate at the National Council on Aging. In this capacity, she advocates for older Americans at the federal policy level, focusing on critical issues such as the Older Americans Act, mental health, vaccine equity, falls prevention as well as other healthcare, equity, and retirement issues. 

    Prior to starting her professional career at NCOA in September 2023, Sophie served as Health Policy Intern at Powers Pyles Sutter and Verville, a Washington, DC-based law firm focused on healthcare issues. In this role, she supported the Powers team in various policy areas including aging, disability, and maternal health. During the Fall of 2021, Sophie served as a Legislative Intern for the United States Senate.

    Sophie holds a Master of Public Health (MPH) degree, with a health policy focus, from The George Washington University Milken Institute School of Public Health. She also received her B.S. in Public Health with a minor in Political Science from West Virginia University.
