As community-based organizations are scaling evidence-based programs and striving to reach underserved populations, additional funding beyond traditional state and federal sources is needed for growth and sustainability. This session will provide an overview of the health care landscape and opportunities for engagement. Community-based organizations will share success with various types of health care payment and reimbursement.

Nikki Kmicinski

Executive Director

Western New York Integrated Care Collaborative, Inc.

Nikki Kmicinski is the Executive Director for the non-profit Western New York Integrated Care Collaborative (WNYICC) where she oversees this regional network of over 20 agencies working toward sustainability and integrated health care. Nikki has been a Registered Dietitian for 20+ years, was a licensed real estate agent for 10 years, and owns a property management company with her husband. She is a Master Trainer for the Self-Management Resource Center’s Self-Management Programs, and a Diabetes Prevention Program Lifestyle Coach. Nikki has a Master’s degree from Rochester Institute of Technology in Service Management and throughout her professional career she has worked in numerous healthcare and business settings including: hospital, nursing home, restaurant, private practice, real estate, property management, business owner, school, amusement park, hotel, municipal Area Agency on Aging, and non-profit community-based organizations.

Caitlin Buechley, MPH

Senior Program Coordinator

National Kidney Foundation of Michigan

Caitlin Buechley, MPH is a Senior Program Coordinator in the Patient Services department at the NKFM. As a Leader and Master Trainer for CDSMP, she has 5 years of experience facilitating community-based workshops and leader trainings. Her purpose is to support individuals living with kidney disease, other chronic health conditions, and disabilities in becoming active self-managers of their health.

Ann Andrews, MPH

Senior Program Manager

National Kidney Foundation of Michigan

Ann Andrews, MPH is knowledgeable about business development with respect to CDSMP, Better Choices, Better Health, Enhance Fitness, and A Matter of Balance and has provided oversight to seven federal grants over the past 16 years. Ms. Andrews has extensive experience developing and sustaining relationships with physicians, hospitals, and insurers.  She is a trained leader for both the Building Better Caregivers and Powerful Tools for Caregivers programs.

Jodi Burke, MSW

Program Specialist

National Kidney Foundation of Michigan

Jodi Burke, MSW has 14 years of grant management experience, including working with Area Agencies on Aging and local health systems to provide evidence-based services, state supported efforts around diabetes management and worksite wellness, and federal initiatives focused on CDSMP expansion and improving women’s health. She has extensive experience in chronic disease management and prevention, as well as partner development and coalition building. Ms. Burke is a master trainer in CDSMP, DSMP, Chronic Pain Self-Management (in-person and virtual), Building Better Caregivers and the Powerful Tools for Caregivers programs. She is also a Diabetes Prevention program lifestyle coach.

Sharon Williams (Moderator)

Sharon R. Williams is the founder and CEO of Williams Jaxon, Consulting, LLC, a business development and training firm focused on support for community organizations, government agencies and health organizations serving Medicare/Medicare consumers. Sharon’s experience includes executive leadership in both the traditional health care and the Home and Community Based Services industries.  She is a seasoned and results oriented executive with a passion for the delivery of high quality, culturally competent, comprehensive and cost effective health care services for publicly funded consumers. Sharon was CEO at CareSource Michigan, a top 100 ranked health plan with Medicaid and Medicare product lines.  Additional leadership experiences include Vice President roles at Coventry, Amerigroup and OmniCare.  She has also served as COO for one of the nation’s leading Area Agencies on Aging.
