The majority of a person’s health is influenced by factors outside the clinical setting – such as where someone lives or works or their socioeconomic status – called social determinants of health (SDOH). There is growing recognition among health payers of the impact of SDOH on a person’s health and wellbeing. Leveraging the NLE’s experience with commercial and Medicare Advantage organizations, the MA4 Network instituted a statewide SDOH screening and referral process to local community support resources for all participants enrolled in evidence-based programs throughout the MA4 Network.

Victoria Jackson, MS is an Integrated Care Specialist with the Mid-America Regional Council (MARC), Managed Services Network (MSN) Coordinator, Evidence-Based Program (EBP) Coordinator, Master Trainer and Leader of CDSME Programs, and a Leader for Walk with Ease and Active Living Every Day. She supports both a regional Community Integrated Health Network (CIHN) of community-based organizations who deliver an array of support services in care management, supplemental meals, transportation, and EBPs and the Missouri Association of Area Agencies on Aging (ma4) Network's statewide delivery of EBPs and person-centered supports for social determinants of health (SDOH). She is committed to creating a healthier community by fostering community connections, providing quality health and wellness programs, and strengthening support services.

Erika is a consultant focusing on design, implementation, and management of programs to improve health care delivery to marginalized and underserved communities. Erika supported the Mid-America Regional Council to establish a Community Care Hub operating in both a regional and statewide market, including network infrastructure development, contracting, quality assurance, and referral management.

Tane Lewis

Managed Services Network Supervisor

Tane Lewis, BA, Evidence-Based Program (EBP) Coordinator for Mid-America Regional Center (MARC), Local Project Coordinator for the Missouri Association of Area Agencies on Aging (MA4), Master Trainer and Leader of Chronic Disease Self-Management Education (CDSME) Programs, and Community Health Worker (CHW). Tane is also a member of the Missouri Senior Falls Prevention Coalition of Clay and Platte Counties. As the EBP Coordinator for MARC Tane is building a regional network of Community Based Organizations (CBOs), in partnership with MARC, that are committed to facilitating EBPs to empower our communities through health promotion, disease prevention education and support services. MARC is dedicated to investing in local CBOs to increase the capacity and sustainability of their health and wellness programs to better serve our communities.

Xavier Vaughn (Moderator)

Program Manager of Medicare

Xavier Vaughn is a Program Manager of Medicare at the AARP. He also provided technical assistance for Benefits Enrollment Centers and Senior SNAP Grantees in the Midwest and Southeast. Prior to AARP and NCOA, Vaughn was the MIPPA Coordinator for the SHIP in Missouri. He currently serves on the board for the Paula J. Carter Center on Minority Health and Aging at Lincoln University. Vaughn received his Bachelor of Science in Health Science from Truman State University and his Master of Public Health from the University of Alabama – Birmingham.
