Building Blocks for Participating in a Coordinated Network of Community-Based Organizations

Eighty percent of ACL falls prevention and chronic disease self-management education (CDSME) grantee respondents on the 2022 Business Acumen and Sustainability Assessment are interested in participating in a coordinated network of community-based organizations (CBOs). However, many are not currently participating in a network or are just starting to explore options. There is a need among ACL grantees and other community-based organizations to understand the role of network participants/service delivery providers and to acquire the knowledge and skills needed to adhere to contracts managed by Community Care Hubs. This four-part webinar series highlights different components to serve as an effective partner in a coordinated network.

Kathryn has experience in community and social services program management, partnership development, coalition building, logistics and operations management, budget and resource allocation, and problem solving.

He has more than 25 years of leadership and professional experience in managed care, advocacy, health policy, small business development, and business acumen consulting for CBOs. Cantrell’s professional positions have included: vice president of product development for the Georgia Business Forum on Health/Atlanta Health Care Alliance, director of the Life and Health Division and health policy lead for the Georgia Department of Insurance, and 15 years of market and corporate level positions with a national health plan. In 2012, Paul founded Clear View Strategies to focus on improving business acumen and results at the intersection of health and social care to support CBOs and health care organizations as they address SDoH and health equity for the individuals in the communities they serve. Paul also serves as the Executive Director of Comprehensive Care Connections (C3), a non-profit Network Lead Entity (NLE) supporting Pennsylvania’s AAAs and their state association (P4A).

Tane is also a member of the Missouri Senior Falls Prevention Coalition of Clay and Platte Counties. As the EBP Coordinator for MARC Tane is building a regional network of Community Based Organizations (CBOs), in partnership with MARC, that are committed to facilitating EBPs to empower our communities through health promotion, disease prevention education and support services. MARC is dedicated to investing in local CBOs to increase the capacity and sustainability of their health and wellness programs to better serve our communities.

Nikki has been a Registered Dietitian for 20+ years, was a licensed real estate agent for 10 years, and owns a property management company with her husband. She is a Master Trainer for the Self-Management Resource Center’s Self-Management Programs, and a Diabetes Prevention Program Lifestyle Coach. Nikki has a Master’s degree from Rochester Institute of Technology in Service Management and throughout her professional career she has worked in numerous healthcare and business settings including: hospital, nursing home, restaurant, private practice, real estate, property management, business owner, school, amusement park, hotel, municipal Area Agency on Aging, and non-profit community-based organizations.

    A career leader in the public sector and human services, Laurie Pferr is a highly regarded subject matter expert in the aging and disability services ecosystem. She is recognized as an adviser and partner who sparks, designs, and guides impactful and transformative programs and advocacy training from inception through delivery. Her political acumen and social determinants of health knowledge are critical and highly valued components of her background. From concept to execution, Laurie has worked on initiatives such as establishing the NY Connects No Wrong Door System and building out local delivery of evidence based self-management programs across the aging, health, and disability networks. Laurie performs with a collaborative style, negotiating skill, creativity, and determination, with a consistent eye on process. Prior to serving as Deputy Director for Policy, Planning, Program and Outcomes at the NYS Office for the Aging (NYSOFA), she held several key senior positions in the agency. For almost twenty years, Laurie worked in the NYS Senate, where she served as Director of the State Aging Committee for five years. Laurie holds a B.A. in Political Studies from Adelphi University and a M.P.A. from Rockefeller College at the State University of New York at Albany.
