Introduction and Training: Healthy Aging Programs Integrated Database™
Overview
Learn more about the data collection and reporting requirements for 2024 grantees. This includes an orientation of the new Healthy Aging Program Integrated Database (HAPID), such as getting set up, finding resources and help, navigating the database, and entering workshops. A separate webinar will be scheduled in the Fall to review reports and dashboards. The intended audience is ACL grantees awarded in June 2024.

Join this webinar for an orientation of the new Healthy Aging Program Integrated Database (HAPID), including how to navigate the system, create new workshops, add host and implementation sites, track workshop activities, and produce reports. HAPI-D will provide a one-stop shop for all grantees and database users to enter workshop data and track performance for both CDSME and falls prevention evidence-based programs. The integrated database combined the National Council on Aging’s National CDSME and Falls Prevention databases, which have been separate entities for nearly a decade, entirely on NCOA’s Salesforce platform.

The Administration for Community Living Prevention and Public Health Fund grants for chronic disease self-management education and falls prevention require awardees to utilize approved data collection tools for all programming. Join this webinar to learn more about the data collection and reporting requirements for 2023 grantees. NCOA and Collaborative Consulting will present an assessment opportunity for Administration for Community Living Chronic Disease Self-Management Education and Falls Prevention grantees focused on better understanding your organization’s capability to achieve various sustainability strategies. This assessment asks questions about your current experience and expertise around leadership, strategy & planning, administration, program management, market intelligence & orientation, partnership development, and financial acumen. The responses to the assessment questions can be used to understand your current state and consider where you would like to be in the future. Aggregate results will be shared with all and NCOA will use responses to inform future technical assistance on these topics. The assessment was available for Administration for Community Living Chronic Disease Self-Management Education and Falls Prevention grantees to complete in March in order to better understand your organization’s capability to achieve various sustainability strategies around leadership, strategy & planning, administration, program management, market intelligence & orientation, partnership development, and financial acumen. This webinar will share the aggregate results from 37 organizations that completed the assessment, trends in strengths and areas for improvement, and how you can use your organization’s results to advance your evidence-based programming sustainability strategies.

Reducing Fall Risks at Home: How to Integrate Home Safety Services into Existing Falls Prevention Efforts
Overview
Preventing falls requires a comprehensive multi-factorial approach to reduce risk factors among community-dwelling older adults. Adding or expanding efforts related to home safety and modifications is key to a multi-factorial strategy. This webinar is intended for ACL Falls Prevention grantees to explore how they can build or expand capacity for home safety/modification services to augment their evidence-based programs. An overview of home modification components, such as assessments, key partners to engage, and examples of simple to complex home modification interventions will be provided during the webinar. A summary of home modification research, programs/promising practices, funding sources, and available resources will be described. The webinar is provided by NCOA’s National Falls Prevention Resource Center, in partnership with the University of Southern California’s Fall Prevention Center of Excellence.
Objectives:
At the end of the webinar, ACL Falls Prevention grantees will:
- Understand what capacity is necessary to play a role in offering home safety/modification services and programs
- Identify examples of programs or promising practices that would be appropriate for implementation within their communities
- Determine next steps to take and partners to engage with the goal of expanding falls prevention interventions that address home safety and home modifications

Since 2019, NCOA has partnered in various capacities with the U.S. Census Bureau, to ensure that all older adults and adults with disabilities are counted. Our partnership has included messaging, outreach, and toolkits built specifically to inform the aging network on how to fairly and accurately participate in the U.S. Census. This virtual training continues our partnership and the importance of data. Census subject matter experts will provide an overview of Census data tools that contribute to a more holistic picture of how individuals are aging in the U.S. Hosted by NCOA’s Equity in Aging Collaborative, subject matter experts from the U.S. Census Bureau will discuss demographics related to older adults, to include: household composition of grandparents living with and responsible for their grandchildren; and data sets within the intersecting topics of aging, disability and poverty. Statistics like these inform researchers, educators, advocates, and policy makers, and can help us work together to prepare for demographic shifts among older adults in the coming decade. The webinar will conclude with a brief demonstration on how to access data tables using data.census.gov and Q&A.
