Not Just for Teens... Seniors Go Online to Improve Health

Overview

Eat Smart, Move More, Weigh Less is a 15-week online weight management program offered through synchronous distance-education and includes 15 weekly online lessons led by a Registered Dietitian Nutritionist.  Each live lesson covers an evidence-based healthy eating or physical activity strategy.  The program meets the Administration for Community Living definition of an ‘evidence-based program’ designed to improve the health and well-being, and reduce disease and injury in older adults ages 60 and older and is therefore eligible for use of Older Americans Act funds under Title III-D.  Since 2017, 532 participants 60+ from 8 states and 25+ organizations have enrolled in the program using Title IIID funding. 76% of these participants have completed the program.  The average weight loss for all 60+participants who have participated in the program is 7.2 pounds, the same as the overall average weight loss for all participants, regardless of age.

Building a Person-Centered Enrollment Hub for Medicare-Medicaid Eligible Individuals

This session will share new research findings and policy recommendations regarding the integrated care enrollment and education ecosystem for Medicare-Medicaid eligible people (duals) and enrollment assisters. 

Audience members will achieve the following learning outcomes:

  • Learn about new research that directly engaged enrollment assisters and dually eligible people regarding their perspective on the outreach, enrollment, and education landscape and vision for an improved outreach, enrollment, and education ecosystem
  • Understand strategies for better supporting Medicare-Medicaid enrollment assistance
  • Understand state and federal policy recommendations for building a federal integrated care enrollment resource hub and in-person assistance 
  • Visualize a path forward for the future of integrated care enrollment policy change

America’s growing older adult population is increasingly more racially and culturally diverse, yet the topic of race relations is not addressed by many programs serving older adults. However, it is a critically important issue as our diverse populations age and interact in senior centers and other social service agencies nationwide. Few programs explicitly address racial and cultural sharing and diversity, which only exacerbates the divisions between older adults caused by segregation, prejudice, and structural racism.
