This call will offer participants an opportunity to learn how grantees are offering evidence-based programs during the coronavirus pandemic. In grand rounds style, grantees will share how they are delivering EBPs in the virtual/remote environment, successes of their work, and lessons learned. Join the call to learn about virtual delivery of EBPs, collaborate with others across the country, and ask questions to help your organization work towards offering and/or improving your EBP virtual delivery.

Tracy Carver, MPA serves as Comagine Health’s Sr. Director, Community Health. Tracy leads Comagine Health’s Community Health Service Line, one of four Service Lines in the Systemwide Quality Improvement Division. She has been working in public health and social services for twenty years. She has over fifteen years of experience developing and leading large scale, complex federal health initiatives. She is an experienced Principal Investigator on federal cooperative agreements and a trusted national thought leader in the areas of chronic disease prevention and management, community centered care coordination models, and community care hubs. Tracy is currently co-chair for the Civitas Networks For Health Social Determinants of Health (SDOH) and Interoperability Workgroup, on the Board of Directors for 211Info serving Oregon and SW Washington, a member of the Partnership to Align Social Care Community Care Hub Workgroup, and a member of the National Center for Chronic Disease Prevention and Health Promotion (NCCDPHP), Centers for Disease Control and Prevention (CDC) new Lifestyle Change Implementation Research Network (LCIRN).
