Milford Wellness Village is a repurposed 265,000 square foot former community hospital site, transformed to provide an array of services to meet the comprehensive needs of a community through all stages of its residents’ lives. With nearly 600 employees from the local area, MWV is creating economic vitality, and offering a one-stop service hub, but more important, it’s not just waiting for people to arrive on site. MWV is deploying nurses directly into the community to help meet the needs of elders whose access may be impaired by transportation, but whose wellbeing is equally important as that of other, more mobile, age groups. This session will share insights to the partnerships required to address a growing community’s overall needs, and strategies to most effectively reach and serve high-risk seniors within that community.
At the end of this session, participants will be able to:
- Describe where and how a Healthy Village could most benefit your community and who may be the most effective partners in starting the conversation.
- Define a framework of partners who can build and deploy a non-traditional strategy to most effectively reach the highest risk target audience of homebound elderly.
- Identify a nurse-led strategy to engage the greater community in ongoing, sustainable, education and services that nurture health and wellness.

Michelle Mai, MPH is the Senior Program Associate with the Center for Healthy Aging. She brings experience conducting outreach increasing public health education within the community. In this role with NCOA, she provides technical assistance to chronic disease self-management education and falls prevention grantees, striving to help them reach their grant goals as they implement evidence based programs into their community. Michelle received her Master’s degree in Public Health and Bachelor’s degree in Community Health from George Mason University.

Alonzo “Lon” Kieffer is an accomplished senior services manager, with niche expertise administering Continuing Care Retirement Communities using an education-based, proactive person-centered model. A licensed Nursing Home Administrator, Lon was awarded Administration of the Year by the DE Health Care Facilities Organization in 2004. In his “free time”, Lon owns and operates a Caregiver-focused organization providing training through humor-based speaking engagements and presentations. Lon is dual degreed with a BSN and MBA.
Kemi Sanni DNP, MSN, APRN, FNP-BC, CRNP

Dr. Sanni is a driven nursing professional with many years of diverse experience in a variety of settings ranging from acute care, long-term care, and in academics as a clinical nursing instructor. Dr. Sanni brings enjoys a long history of nursing leadership in the development of educational programs, curricula, and lesson plans for nursing students, nursing staff, and consumers. Her focus in clinical nursing practice, and at bedside, has been in the development and management of patient-focused care plans. Dr. Sanni earned her Doctorate of Nursing Practice from Walden University and her Bachelor of Science in Nursing from Delaware State University. Dr. Sanni also has two Masters of Sciences in Nursing degrees, one for family nurse practitioner and the other, Nursing Informatics. She has been in the health and wellness profession for 21 years, starting as a Staff Nurse at the Washington Hospital Center, Washington, DC, staff nursing in cardiac and medical surgical units in local Delaware hospitals (Bayhealth Medical Center and Nanticoke Memorial Hospital), and most recently as Director of Nurse Practice Education and Staff Development at Genesis. Dr. Sanni maintains numerous professional affiliations.
