P5: Deep Dive: Healthier You: Long-term Benefits of Bundling Multiple Programs and Resources

Healthier You is a 6-month program to reduce social isolation and food insecurity and encourage physical activity, especially for obese older adults. This presentation will describe how a rural site identified strategies to address these related health issues among a diverse group of older adults. Healthier You participants received weekly cartons of healthy vegetables, 5G tablets to link to virtual programs and family/friends via the internet, and access to a physical activity program in-person or via Zoom. They also received scales so they could measure and record their weight loss. This interactive session will discuss techniques to recruit diverse participants, share experiences about ‘bundling’ services and programs for maximum impact, and explain the program’s effectiveness. 

At the end of this session, participants will be able to: 

  • Identify strategies to recruit diverse participants in programs and services.
  • Discuss benefits of bundling several programs and services to improve participant outcomes.
  • Identify strategies to encourage full participation of individuals over an extended period.

Leigh Ann Eagle is the Chief Operating Officer of the Living Well Center of Excellence (LWCE) at MAC, Inc., an area agency on aging that oversees Maryland’s implementation of Healthy Aging Evidence Based Programs. Ms. Eagle is responsible for all aspects of program management for LWCE. She created the Mid Shore Coalition for Chronic Disease. Under Leigh Ann's leadership, Living Well is connecting healthcare organizations with aging and community resources to coordinate statewide program delivery. LWCE is a one of twelve National Network Lead Entities funded by the U.S. Administration for Community Living.

 Sue oversees statewide implementation of evidence-based programs; assists in grant-writing, partnership/capacity building; provides leader trainings for various evidence-based wellness programs and implements evidence-based community workshopsShe collaborates with healthcare systems to embed and sustain evidence-based behavior change programs into care delivery. Sue was previously the Senior Director of the National Council on Aging Chronic Disease Self-Management Resource Center where she led a team of professionals in providing national technical assistance to 47 state agencies. Prior to that, Sue oversaw evidence-based program implementation across New Jersey for the Department of Health and Senior Services and was previously a lobbyist for the Alzheimer’s Association in New Jersey. 

Matthew Smith PhD, MPH, CHES

Dr. Matthew Lee Smith is an associate professor in the Texas A&M School of Public Health and primary faculty member of in the Texas A&M Health Center for Population Health and Aging.  As an evaluator and interventionist, his research addresses health risk across the life-course and the uptake of evidence-based solutions for older adults. Dr. Smith has devoted his career to creating synergistic partnerships and initiatives to encourage positive lifestyles and reduce preventable morbidity and mortality. His translational work bridges research and practice issues across the healthcare sector, aging services network, and public health system.
