Partnerships play a critical role in falls prevention and home modification services to support aging in place and community living. This webinar is designed to connect ACL grantees (funded by the Administration for Community Living to implement evidence-based falls prevention programs) with grantees of the Older Adult Home Modification Program (funded by the U.S. Department of Housing and Urban Development to provide home modifications for older homeowners). Participants will learn about each group of grantees and how partnerships can build capacity and augment their grant-funded efforts. Examples of partnerships that support home modification and falls prevention services will be highlighted, and an ACL grantee and HUD grantee will share first-hand accounts of how partnerships strengthen their work and address challenges in their communities. Dedicated time in breakout rooms will enable participants to connect with other grantees in their geographic region and lay the groundwork for future collaboration. Resources to support partnership building, home modification, and falls prevention service coordination will also be shared.
Objectives: At the end of the webinar, participants will:
- Identify partners to engage with the goal of expanding falls prevention and home modification interventions
- Be prepared to develop partnerships that improve home modification and falls prevention service coordination
- Understand common challenges related to implementing falls prevention and home modification programs in diverse communities across the country
