    Selene Phuong

    Project Coordinator

    Selene has been a Project Coordinator with Partners in Care since September 2022. She received her Bachelor’s in both Public Health Policy and Urban Studies at University California Irvine in 2022. Selene has volunteered and interned with organizations such as American Heart Association and St. Jude Wellness Center during her college years that has gave her the opportunity to learn about integrative lifestyle approaches to help others improve their health conditions. In her career, Selene hopes to be able to make health resources more accessible to disadvantaged communities. On her days off, Selene enjoys exploring new foodie and drink places with her friends and family.

    Rolando Toledo

    Project Supervisor, Community Wellness Department

    Partners in Care Foundation

    Rolando Toledo, MPH, has contributed to various organizations as a Health Educator, Program Coordinator, and Project Supervisor. He has worked in Public Health, community clinics, and health plan settings. He enjoys promoting health and wellness strategies throughout the continuum of life. He values his engagement in multidisciplinary teams working towards health equity, access, and optimal social and healthcare outcomes. Working with diverse populations, learning from different experiences, and sharing common bonds brings him fulfillment. Currently, he assists in the planning, delivery, and coordination of evidence-based wellness programs for older adults and adults with disabilities. Rolando enjoys traveling with his family and learning about humor, faith, and positive psychology influence on health, longevity, and wellbeing. 

    Yoko Meusch, MA

    Program Associate, Center for Healthy Aging

    Yoko Meusch, MA, is a Senior Program Specialist with more than five years of dedicated service to the National Council on Aging’s Center for Healthy Aging (CHA).

    In her role, she leads national, grant-funded initiatives designed to strengthen community-based aging programs, improve health outcomes, and broaden equitable access to evidence-based interventions. Yoko provides comprehensive project management and technical assistance while fostering strategic collaboration with funders, researchers, advisory groups, and community organizations. She brings extensive experience in facilitating learning collaboratives, developing practitioner-focused resources, and enhancing delivery infrastructures that support healthier aging nationwide.

    Yoko holds a Master of Arts in Communication with a concentration in Health Communication from Johns Hopkins University. She is a military spouse and a mother of three free-spirited, energetic children.

    Joani is a Program Manager with the Training and Capacity Building Program within the Health Promotion Council, an entity of Public Health Management Corporation (PHMC) in Philadelphia. She is responsible for overseeing and assisting organizations with implementing programs, including the National Diabetes Prevention Program, DSMES, Walk With Ease, and Self-Management Resource Center programming. She does this through community engagement, technical assistance, data support, and guidance recommendations.
