Yoko Meusch, MA, is a Program Associate for the Center for Healthy Aging at National Council on Aging. Yoko provides technical assistance to U.S. Administration for Community Living (ACL) grant-funded community-based organizations promoting and implementing evidence-based Falls Prevention and Chronic Disease Self-Management education programs. In addition, she is a program administrator for Aging Mastery Program®, an NCOA’s signature program for aging well.
Yoko holds a Bachelor's degree in Management Studies from the University of Maryland Global Campus and a Master's Degree in Health Communication from Johns Hopkins University. -

Stony Brook Southampton Hospital’s Center for Parkinson’s Disease and Falls Prevention Program
Sarah designed and launched the Center for Parkinson’s Disease and won the 2019 Degenerative Diseases Special Interest Group Service Award in recognition of her “tireless pursuit of improving the lives of individuals with [PD] who live on the East End of Long Island.” She oversees seven PD-specific wellness programs, all currently implemented remotely, with over 250 participants. She is a co-PI on the 2022-2025 ACL Falls Prevention Grant.
Kristi Ladowski, MPH
Injury Prevention & Outreach Coordinator
Stony Brook University Hospital Trauma Center
Kristi has over 10 years of experience coordinating health promotion programs, leading the efforts that earned the 2020 Safe States Injury Prevention Program of the Year Award. She chairs state and national hospital-based/trauma center injury prevention subcommittees. She served as a Regional Coordinator for previous ACL Falls Prevention grants before becoming a co-PI for the 2022-2025 cycle.

Jill Renken, MPH, CHES

Jill Renken has a bachelor’s degree in Health Promotion & Wellness and Master of Public Health degree. She is also a Certified Health Education Specialist. Jill has done health education work in the university, healthcare, and worksite employee wellness setting, with most experience in community wellness programs. She currently works within Wisconsin’s Aging Network and Partner Organizations to oversee statewide implementation of Evidence-Based Healthy Aging Programs. Over the past 15 years, Jill has provided technical assistance and training around evidence-based health promotion programs throughout the state. Currently, as the Director of Program Implementation for the Wisconsin Institute for Healthy Aging (WIHA), she works to develop and support strategic organizational partnerships that assist in expanding and sustaining the dissemination of evidence-based community health promotion programs to prevent disease, reduce emergency department visits, hospitalizations and falls, and improve health and quality of life of older adults in Wisconsin and Nationwide.
